Support and outreach services may include the following: • Collaborate with City of Minneapolis Neighborhood Safety Department (NSD) staff on resource identification and building collaborative relationships with agencies/individuals working on the issue of violence prevention/intervention with group/clique involved youth. • Develop stakeholder network by building relationships with community members, community leaders, and faith leaders to aid in removing obstacles that typically prohibited group involved youth from receiving help and building legitimate community relationships. • Receive referrals to work with group involved youth from agencies/individuals, including Hennepin County parole and probation, Minneapolis Police Department (MPD) and other sources. • Build rapport and positive relationships with youth. • Discuss goals, create goal plans, and support youth’ success toward goals related to helping them exit cycles of violence and keep them safe, alive, free. • Identify resources for basic needs, resources, and services needed to help youth exit cycles of violence and keep them safe, alive, and free. • Attend YGVI staff and coordination meetings and participate in opportunities to convey YGVI message (including intake and check ins). • Engage in regular meetings with YGVI project manager to review cases and provide regular reporting on youth engagement, services provided, challenges, success stories, etc. • Partner with community organizations based on finalized asset mapping plan. • Send biweekly or as required updates to Program manager
