Selected contractors will provide the following services to all MHC candidates referred by WCHCS’ MHC Program Manager: A. Activate Medicaid or other healthcare benefits for members who qualify for those plans; B. Assessment services are intended to provide a smooth transition to community-based services for the MHC member upon re-entry from the Jail. The Contractor will: 1. Complete one or more of the following assessments for individuals detained in Whatcom County Jail: a. Social Determinants of Health (SDH). SDH assessments must be completed before release in a mutually agreed upon time frame with the MHC program supervisor; b. Mental Health. Mental Health assessments must be completed before release in a mutually agreed upon time frame with the MHC program supervisor; c. Substance Use Disorder (SUD). SUD assessments must be completed within 10 working days of the request; 2. Following assessment completion, ensure member transition from the Jail to outpatient services, primary care homes with an overview of care coordination needs based on SDH and Behavioral Health Treatment recommendations, and communicate with the MHC Program Manager regarding activities. a. If the Contractor is unable to place members in services due to agency capacity, individuals will be placed on a priority waitlist for services and referral will remain valid for 90 days; C. Provide progress reports (both verbal and written), including progress on MHC phases and significant events, and documentation in shared electronic records, as required by the MHC Program Manager for weekly or bi-weekly court dockets;
