Proposals should consider and address how the work within each project is addressing Individual, institutional and systemic change. • Individually, the work should address how individual agencies/organizations can be affected, improved upon and provide for them a better understanding of how they fit into the larger institution and system. • Institutionally, the work should improve the policies, best practices and procedures of the larger organization, known as the Continuum of Care, while providing for membership growth, understanding and openness. • Systemically, project work will provide a pathway for CoC growth and understanding of how the system is a part of the larger environment, and to be successful the CoC will need to improve and build positive interactions with other systems of care, other institutions, and take a broader more regional approach to partners, partnerships, collaborations, etc. to emphasize overall policy framework development, implementation, improvement, operations and interactions...
