Contractor Requirements The Parent Advisor(s) shall provide the following service activities as appropriately determined by the individual needs of the client. 1. The Parent Advisor (s) shall provide special instruction in the child and family’s natural environment. Design learning environments and activities that promote the infant’s or toddler’s acquisition of skills in a variety of developmental areas including cognitive processes and social interaction. Utilize and update prescribed lesson plans that reflect the unique needs of the child with sensory disabilities. 2. The Parent Advisor (s) shall conduct curriculum-based assessments and establish reasonable outcomes for the child with the Individual Family Service Plan (IFSP) team. Outcomes will include curriculum-based planning to include the planned interaction of personnel, materials, time, and space that leads to achieving the outcomes in the Individual Family Service Plan (IFSP) for the toddler or infant with sensory disability. 3. The Parent Advisor (s) shall provide families and other Individual Family Service Plan (IFSP) team members with information, skills, and support related to enhancing the skill development of the child. Work with the infant or toddler to model best practices and instructional strategies for the family and Individual Family Service Plan (IFSP) team members. Participate in transition/exit plan from Early Intervention services. Provide educational options to the family which include communication options, use of assistive technology and other unique needs of the child. 4. The Parent Advisor (s) shall document services rendered of the child and family. The document forms to submit are the Travel Log, Family Training Log, and the Family Training Signature Sheet. All documents must be submitted by the 5th of each month to ensure case records are kept up-to-date, organized, and accurate. Submit all documentation by mail to: Early Intervention Outreach Office, 101 Executive Center Drive, Suite 120- Saluda Building, Columbia, SC 29210. 5. The Parent Advisor (s) shall record activities and data related to services on a Monthly Log/Pay Voucher. 6. The Parent Advisor (s) shall provide their own transportation to travel and perform family training duties. 7. The Parent Advisor (s) shall adhere to the Individualized Family Service Plan (IFSP) and all pertinent information to serve the family, which will be given to the Parent Advisor. 8. The Parent Advisor (s) shall provide services only to the client’s family which has been assigned to them by the Early Intervention Program Supervisor.
