Phase 1 – Evaluation Initiation: 30 Days – Approx. April 15-May 15, 2025 Develop a comprehensive evaluation plan that aligns with the project’s goals and objectives and aligns with federal evaluation guidelines. Review the grant project's goals, objectives, and outcomes as well as the project's activities, timeline, and budget. Become familiar with the ASURams Global leadership and staff and outline a standard of collaboration with ASURams Global throughout the project. Understand the target population and their specific needs. Produce an Evaluation Plan that includes timeline, strategies for data collection, and analysis methodologies. Evaluation Plan should also include acknowledgement and a plan to ensure drafts of the Final Grant Report and Replication Manual are completed ahead of Closeout Date. Deliverables: Comprehensive Evaluation Plan Phase 2 – Data Collection: 150 Days – Approx. May 15-Oct. 15, 2025 Develop data collection instruments, implement a data collection activities that are compliant with appropriate regulations, ethical, and uses relevant methodologies. Design and implement evaluation data collection Plan with procedures and timelines. Collect data according to Evaluation and Data Collection Plan from relevant stakeholders such as: Grant recipients, project participants, community leaders, and program staff/leadership. Remain in contact with ASURams Global staff and leadership about data collection needs Deliverables: Data Collection Instruments and Detailed Data Collection Plan Phase 3 – Analysis and Reporting: 90 Days – Approx. Oct. 15, 2025 -Jan. 15, 2026 Analyze data to assess project progress on CMC Project goals and objectives. Use appropriate methodologies to assess project impact and identify key findings, insights, and best practices for future projects seeking to implement similar strategies. Communicate this evaluation and findings to ASURams Global Staff in both an internal presentation and in writing in the form of a public Final Grant Report and Replication Manual. Analyze data using appropriate methodologies and present data findings in the form of a presentation to ASURams Global. Prepare comprehensive evaluation report that assess progress and completion of CMC goals and objectives and make recommendations for future projects. Report should include: Executive Summary Project Overview Evaluation Data and Methodology Analysis and Findings Conclusions and Recommendations Prepare a Replication Manual for use by future organizations seeking to implement similar projects. Manual should include: Summary of Project’s goals, objectives, and outcomes Description of Project Planning and Implementation Process Recommendations and Best Practices Deliverables: Analysis Findings Presentation to ASURams Global. Written Final Grant Report. Written Replication Manual.
