• Provide optometry services on the campus of Larned State Hospital (LSH) to patients/residents in accordance with applicable standards and requirements. • Provide services on a regularly scheduled basis approximately two days a month (typically will provide 16 hours per month). Dates and times for providing this service will be established monthly through mutual agreement between the contractor and LSH. If LSH requires clinic closures, contractor shall be provided with an opportunity to account for lost service hours on dates and times established through mutual agreement between contractor and LSH. In no event shall the contractor provide more than 192 hours per fiscal year. • Provide routine procedures such as fittings and repairing of glasses, examination of refractory errors, routine eye examination, testing for visual acuity, external eye examination. • Provide other such procedures and/or testing falling within the contractor's scope of practice, such as Goldmann Applanation tonometry, and dilated internal fundus of examination. • Contractor shall communicate to LSH medical staff any recommendation for referral of patient treatment not available at LSH.
