Based on data analysis, provider surveys, interviews of persons who have received FHPAP services, and an FHPAP design workshop held on August 22, 2024, the Heading Home Washington FHPAP Advisory Board elects to focus its 2025-2027 Request for Proposal on the following three service models: • Coordinated Entry/Diversion, with a focus on sustained communication with assessed households and support to help them prepare for a housing opportunity. • Homelessness Prevention, which is essential to keep households from becoming homeless. • Medium Term Rapid Rehousing to support assessed households, prioritizing those with a clear connection to Washington County, quickly reenter housing and find support and necessary rental assistance.
