The STATE seeks to provide grant funding to engage and connect people from historically underserved cultural and ethnic communities and/or people who live in rural Minnesota who are eligible for HCBS through outreach and service development activities. STATE is particularly interested in supporting older adults and people with disabilities where English is not their primary language and may not be familiar with the HCBS service system. Bilingual or multilingual organizations and culturally specific community-based organizations are encouraged to apply. Examples of eligible activities under this grant include, but are not limited to: Developing outreach materials tailored to the specific racial/ethnic/cultural/disability/LGBTQ+ community or communities (e.g., culturally relevant, linguistically appropriate, accessible, etc.), Creating an awareness to the availability of HCBS services in the area through outreach by using materials developed and targeting and engaging communities, Holding or attending community events to learn from/educate community members about HCBS, and connect people to HCBS services by working with providers, the STATE and its partners, Connecting people to qualified service providers who can provide HCBS in the area and provide accurate information and resources related to HCBS, Developing media messaging that are culturally and linguistically appropriate with information about HCBS and a list of providers in the area that are available to provide services that reaches large target audiences. Such media messaging may include social media, online news, radio, print brochures, and other outreach resources that are adaptable with information on available services, Developing partnerships with trusted community and/or faith leaders to outreach to people in the community.
