Please note: This is a Sole Source Notification. Specifications include, but are not limited to: (2) complete a mental health diagnostic assessment for each child, prior to their admission to the Day Treatment Program, by a licensed mental health provider. The assessment must include diagnostic formulation and developmentally appropriate diagnosis (as applicable) to determine clinically indicated level of care. If the child mental health diagnostic assessment does not indicate day treatment level of care, the Contractor shall provide level of care recommendations and provide a warm hand off to the child’s ongoing provider for services at the indicated level of care; (3) provide mental health services to children enrolled in the Day Treatment Program, as clinically indicated, which may include services as defined by the Utah Medicaid Behavioral Health manual. The services are: individual therapy; family therapy; psychosocial rehabilitation services; targeted case management; and other services provided by the Contractor; (4) maintain documentation of services in accordance with Medicaid documentation requirements for each contract funded client; and (5) provide discharge planning in accordance with mental health industry best practices for each contract funded client.
