Please note: This is a Sole Source Notification. Specifications include, but are not limited to: 4.1 Provide a forensic medical evaluation of children suspected of being abused or neglected. 4.1.1 When referrals are received from the ADCS Specialist and/or OCWI Investigator (from 8:00 a.m. to 5:00 p.m. Monday through Friday with the exception of State holidays), the Contractor shall arrange for the child to be examined as determined by a Child Protection Team (CPT). For examinations requested after working hours, the Contractor shall examine the child no later than twelve (12) hours from the time of notification, unless otherwise designated by the CPT. 4.1.2 Order and review laboratory and radiology services as needed. 4.1.3 The investigative physician and/or qualified provider may authorize, following a staffing with the designated ADCS Program Supervisor, one or more of the following laboratory tests or examinations: sexual abuse, physical, Computed Axial Tomography (CAT) Scan Brain or Magnetic Resonance Imaging (MRI) Brain. The MRI Brain and CAT Scan Brain are only to be authorized when there is a concern for brain injury. 4.1.4 Upon completion of the forensic medical examination, immediately provide a verbal update and e-mail a summary report to the designated ADCS Specialist and/or OCWI Investigator. 4.1.5 Prescribe and/or recommend treatment, which may include prescriptions. 4.1.6 If the client has injuries, prepare a Child Abuse or Neglect Report (Exhibit B), and submit to the ADCS designated Specialist and/or OCWI Investigator within forty-eight (48) hours of completion of the examination. The Child Abuse or Neglect Report (Exhibit B) shall include, but not be limited to, the following: 1. An illustration of a child’s body, including front and back, and an illustration of a child’s head, including front, back, left and right sides; 2. Circled body areas that indicate an injury and specify the type of injury (e.g. burn, bruise, laceration, abrasion); 3. Date of examination; 4. Physician name and signature; and 5. Name of child.
