Contractor shall supply approved 39-gallon resealable nestable containers for the biohazardous regulated medical waste with a minimum 40-gauge size and clearly labeled as being bio-hazardous material. The contractor shall disinfect containers prior to reuse by the facility. The disinfecting agent used in this manner is to assure the eradication of any biological agent that may have remained within the container. All processes should be in accordance with the Federal Regulations Title 49, 100-180 OASHA provisions 29 C.F.R. 1910.1030 as adopted by MOSH and the code of Maryland Regulations Title 26.13.11-13, as well as any other applicable laws or regulations regarding the handling, transportation and disposal of bio-hazardous medical waste. Contractor is to pick up bio-hazardous medical waste a minimum of one (1) time per week and dispose of said waste in accordance with all applicable state and federal laws and regulations. The day and time of pickup shall be a weekday between the hours of 5:00 a.m. and 11:00 a.m. Local Time and shall be agreed upon by the Contractor and the Contract Monitor. Contractor shall provide manifests to document the number of containers and weight of each container at pickup and leave a copy with the contract Monitor or their designee at the time of pickup. The containers will be weighted again by the Contractor prior to loading upon the truck for disposal. Manifest forms shall be provided and originated by the Contractor. Manifest will be completed by DHHC staff, and the Contractor shall provide email and, or online access of said manifest. Said manifest with attached invoice shall be provided within seven business days. The contractor shall maintain accurate documentation detailing the destruction and disposal of the picked-up bio-hazardous regulated medical waste. Containers shall be marked KDU (Kidney Dialysis Unit) or Hospital for record keeping purposes. Packaged HEPA filters will be set in the trash room along with the bio-hazardous containers sealed in a red bio-hazardous bag for removal. Transfer of the bio-hazardous regulated medical waste from the generator to the transporter to the disposal facility shall be conducted in an environmentally safe manner. The contractor shall provide competent personnel to attend the vehicle at all times during the loading and unloading operations. If for any reason the vehicle operator leaves or is not present at the vehicle, all loading and/or unloading operations shall cease until the operator returns. Should the Contractor cause any contamination of any are in the hospital or hospital grounds, Contractor shall be responsible for prompt decontamination of that area. Decontamination must be conducted according to State and Federal Regulations, and OSHA 29 C.F.R. 1910-1030 as adopted by MOSHA and the Code of Maryland Regulations Title 26.13.11-13, as wall as any other applicable laws or regulations regarding the handling, transportation, and disposal of bio-hazardous regulated medical waste. The contractor must have a Controlled Hazardous Substance Hauler Certificate for each vehicle. Certified vehicles shall display a Special Medical Waste Transporter Vehicle Certification Sticker on the vehicle as required by applicable statue or regulations. All vehicles and equipment are to be in accordance with Federal Regulations Title 49, Subchapter C, Parts 171-180, OSHA provisions 29 C.F.R. 1910-1030 as adopted by MOSHA and the Code of Maryland Regulations Title 26.13.11-13, as well as any other applicable laws or regulations regarding the handling, transportation, and disposal of bio-hazardous regulated medical waste. The Contractor shall provide 10 days prior to the Go-Live Date to the Contract Monitor a copy of each driver’s current Class C Driver’s License. The Contractor’s Vehicle Driver shall compare labels and manifest to ensure that the contents and quantities listed on the manifest are the same as those listed on the containers. The driver shall consult with the Contract Monitor or designee to resolve any discrepancies before removing the involved containers from the facility. In the event of a spill or loss of bio-hazardous regulated medical waste to the environment any Contractor staff engaged in the transportation of bio-hazardous medical waste shall report the incident immediately (within one (1) hour) to MDE by phoning 1-866-633-4686, including holidays and weekends. The person and vehicle involved must remain at the scene and be available until clearance to leave is given by the appropriate officials. A copy of the Hazardous Materials Response Procedures brochure must be carried in the vehicle transport of bio-hazardous regulate medical waste. The contractor shall be solely responsible for any fees, fines, charges, or assessments of any kind associated with or related to a spill, loss, transportation and/or disposal of all bio-hazardous materials picked up and removed from DHHC pursuant to the resulting contract. The Contractor shall be responsible for any fees in connection with the disposal of all bio-hazardous regulated medical waste.
