The Charles County Department of Health Local Behavioral Health Authority requests proposals to acquire a vendor to provide youth and young adults with Emotional and Behavioral Disorders. THe PRogram will provide the following: 1. Engage youth in their person-centered planning and involve youth and their families in a process that prepares and facilitates them in their movement toward greater self-sufficiency and successful achievement of their goals related to relevant transition domains. 2. Assistance in meeting non-mental health needs by providing financial and tother forms of assistance to enable consumers to access transportation, jobs, education and training. 3. Provide case management services that link consumers to all needed services and monitor their progress
