A. The Contractor must collect Title X Family Planning medical data from up to ten (10) delegates from web-based or electronic records for a twelve (12) month period. Data is collected by use of the CVR form (Section III). The completed CVR data will be uploaded to the Contractor from the clinics. A CVR is completed on every Title X client who has a face-to-face or virtual encounter with a program person(s). Upon receipt of the CVR, the Contractor must generate data reports as outlined in the Scope of Work Section V to include site-specific and state-specific quarterly reports to the Department. B. The Contractor must process CVRs received via web-based or electronic record uploads and must review error messages, correct rejections, and resubmit. C. The Contractor must email error messages to the Department and to subrecipients and must make rejected forms and processing summaries available on their website by the fifteenth day of the following month. D. The Contractor must answer emails and phone calls from clinics regarding inquiries for correction of errors, and missing information on reports. E. The Contractor must enroll new Family Planning clinics within a subrecipient’s service area, with approval from the Department, and must maintain records of all facilities reporting, to include purging inactive clinic site numbers. F. The Contractor must maintain updated master databases of CVR visits and facilities, performing updates as necessary, and purging visit information on a quarterly basis. G. The Contractor must maintain a minimum of three (3) years and maximum of five (5) years of the most current historical CVR data and reports and a back-up method for the data and reports in case of system failure...
