The State of Kansas is seeking competitive from qualified vendors to provide Resident Minimum Data Set (MDS) Management services for the Kansas Commission on Veterans Affairs Office – Kansas Soldiers’ Home, per the following specifications. 4.1. MDS Management A. Work with team to schedule, plan and manage MDS completion 4.2. MDS Completion A. Review and ensure MDS’s are completed timely by all the IDT B. Set ARD’s and complete clinical items 4.3. CAA/Care Plan A. Complete Care Area Assessment for items triggered by MDS B. Ensure each participant completes there triggered CAAs C. Provide RN signature of CAA Completion D. Drafted Comprehensive Care Pan with admission MDS i. Baseline care plan is not included 4.4. Case-mix A. CMI management to identify and capture planned or current resources B. Schedule ARD’s 4.5. Daily Management Requirements A. Daily resident Review (DIRECT COMMUNICATIONS): i. PDPM Review and Management ii. OBRA/LTC Review and Management iii. ICD10 Coding for admissions B. Daily Chart Review for resident changes and management C. Daily Orders Review for resident changes and management D. Care Plan review for CAA Information or other concerns E. QIES ASAP Reports and Management F. Survey Assistance for MDS G. Medicare Hotline Long Term Care questions and answers 4.6. Audits A. High level QM reviews Quarterly B. PDPM Financial updates/CMI reviews C. Random Claims review to identify education needs or trends 4.7. Education A. SSD/SS designee MDS/CAA education on request B. Dietary/designee MDS/CAA education on request C. Activities/designee MDS/CAA education on request D. BOM/Designee MDS/PDPM review for billing on request E. PDPM/CaseMix facility training (Putting it all together) on request
