Develop and implement collaborative strategies to address the gap in the linkage. 5) Provide education to clinical and community sector stakeholders by sharing promising practices, highlighting emerging local leaders in the field, and documenting case studies that demonstrate the positive impact on health outcomes as a result of improved Community-Clinical Linkages.
