The focus of this work will be on the kick-off and execution of a defined Platform Element scope set forth in the SHIFT business plan, which include the following work streams: Platform Elements Team-based Care Pilot Readiness Clackamas County’s diverse populations Change Management Infrastructure SHIFT Practice and Financial Modeling Contractor shall provide a designated staff person to perform the following services in support of the work streams identified above. Executive coaching to support SHIFT implementation and improvement iterations Advise County’s project manager(s) in leadership and execution of SHIFT activities Advises County’s SHIFT project leaders through assessment of change to organization and planning of change management strategies and tactics Bring perspectives on outside best practices and industry specific expertise Provide expertise in Lean methodology, process optimization and quality systems Provide advisory services to County’s internal and external stakeholders
