Price agreement for mat and rubber brush mat service at various Street Maintenance Facilities and quantities listed below. Service will be every two weeks. Invoices to be sent once per month and to include all five (5) locations. The initial contract term will be for one (1) year with the option to extend for additional four (4) one-year extensions at the sole discretion of the City of Omaha. Location 1: Administrative Office (3827 S 42 St) will require: 2 – 4x6 mats – (Nylon/Rubber, Steady Step) – dark gray 3 – 3x4 mats (Nylon/Rubber) – dark gray 1 – rubber brush mats Location 2: District 2 yard (5225 Dayton Street) will require: 8 – 4x6 mats – dark gray 13 – 3x10 mats – dark gray 4 – rubber brush mats Location 3: District 3 yard (8750 Vernon Avenue) will require: 3 – 4x6 mats – dark gray 5 – 3x10 mats – dark gray 3 – rubber brush mats Location 4: District 4 yard (4040 S 96th St) will require: 3 – 4x6 mats – dark gray 3 – 3x10 mats – dark gray 3 – rubber brush mats Location 5: District 5 yard (20567 Park Rd, Elkhorn) will require: 1 – 4x6 mat – dark gray 5 – 3x10 mats – dark gray 1 – rubber brush mats
