a. Provide a plan for and cost of snow removal services, as further described in the “Scope of Work” b. Provide evidence that the contractor has at least 3 years of experience providing custodial services to a municipality of comparable size to Boxford. c. Provide evidence that the contractor has at least 8 years of experience providing custodial cleaning services in the public or private sector to facilities of comparable size as the Boxford municipal buildings. d. Provide evidence that the contractor is currently contracted to provide custodial cleaning services including snow removal with a facility of comparable size as Boxford’s municipal buildings (combined) and function in the North Shore area i.e. Essex county. e. Provide a list of current client accounts to include contact name and telephone number, length of service of the account, type of facility and square footage. f. Carry the required amount of insurance as shown by the insurance requirement enclosed herewith. Certification of insurance acceptable to the Town of Boxford shall be provided to the Town of Boxford within 15 days of notice of award. Insurance shall remain in force during the full term of the contractual agreement and/or until work is completed and accepted by the Town of Boxford. The policy must name the Town of Boxford as an additional insured and must state that Town of Boxford will be notified 30 days prior to any change to coverage or lapse in policy. g. Fill out, notarize and include the Non-Collusion Affidavit. h. Failure to comply with any of the above or with any other provision contained in these specifications will disqualify the vendor.
