The State of West Virginia Office of Shared Administration, Dept. of Health Facilities and Hopemont Hospital is accepting bids to establish a contract for a Speech Pathologist to provide Speech Therapy to the residents. 4.1.1 Speech / Language Pathology Services to the residents of Hopemont Hospital. Revised 12/12/2017 4.1.1.1 Vendor must review/ evaluate all new admissions within the first 7 days of admission and bi-annually thereafter or as needed through the consultation process. Current census is 48 which could lower or rise to approximately 98. 4.1.1.2 Vendor must be available for consultation at the request of the agency. 4.1.1.3 Vendor must be available upon request in the event of any Pathology survey (OHFLAC) issues that may arise. 4.1.1.4 Vendor must recommend Dietary adaptive equipment, identify needs for dysphagia treatment, and type of diets, provide dysphagia treatment and speech communication therapy as needs identify. 4.1.1.5 Vendor must provide staff training in-services to the Dietary, Recreation, and Nursing Departments when appropriate. 4.1.1.6 Vendor must re-evaluate all residents with a diagnosis of developmental disability annually. 4.1.1.7 Vendor must re-evaluate any resident upon any identified changes in swallowing or communication status. 4.1.1.8 Vendor must be avail
