1. The Contractor shall operate a no reject acute crisis and evaluation program in a secure facility with a high staff to child/youth ratio as detailed below. The Contractor shall provide individualized support and develop a Plan of Care for each placed child/youth, which shall include the following services: a. Behavioral and emotional support from interventionists; b. Psychiatric monitoring; c. Access to medical care provided by nursing staff; d. Clinical service coordination and case management; e. Behavioral support plan; f. Physical behavior support; and g. Educational materials provided by the child’s/youth’s school of origin. 2. The acute crisis and evaluation program shall serve up to three (3) youth between twelve (12) and eighteen (18) years of age at one time with the possibility to serve up to age nineteen (19) with approval from the FSD Program Manager. The program shall serve all referred youth for up to approximately 45 calendar days, except in the case of emergency removal. The Contractor may serve youth for up to 90 calendar days should an extension be approved by the FSD Program Manager. In times of high need, as requested by FSD, the Contractor shall serve up to three (3) youth at one time. The target population shall be children/youth with serious mental, behavioral, or emotional health concerns, who display behaviors such as: a. Self-injury; b. Suicidal ideation or threats; c.
