2.1 PROJECT OVERVIEW AND HISTORY The STATE intends to enter into a Contract with a qualified Hawaii-based for-profit or notfor-profit community-based agency to provide services that strengthen, support, and mobilize communities through PSE changes rooted in a coordinated approach to promoting physical activity, healthy nutrition, and tobacco-free lifestyles in the STATE, identified by the CDPHPD, as needed. The CDPHPD focuses on PSE changes in various settings (i.e., early care and education (ECE), schools, communities, worksites, and healthcare) and at all levels of government (i.e., local, state, and federal). These changes include supporting organizational and legislative policies that make healthy choices more accessible, promoting built environment changes that incorporate physical activity into daily life, improving access to healthy food for all, and supporting tobacco-free living through PSE changes such as increased regulation, taxation, and smoke-free environments. Obesity and chronic disease prevention initiatives are implemented statewide and in targeted communities using tailored, partner-driven interventions. These initiatives are aligned with the Hawaii Physical Activity and Nutrition Plan 2030 (PAN Plan 2030) and the Hawaii Tobacco Prevention and Control Plan 2030 (TPC Plan 2030), which are the STATE’s guide to increasing access to safe and equitable opportunities for physical activity and healthy foods and decreasing tobacco use. Coalition work also extends to the effective early detection and management of chronic disease as described in the Healthy Hawaii Strategic Plan 2030 (HHSP 2030). This solicitation seeks services that strengthen, support, and mobilize communities through coalition building and PSE changes to increase opportunities and access to physical activity, healthy foods, smoke-free environments, and effective early detection, treatment, and management of chronic disease. Examples of services that may be included are the establishment, maintenance of, recruitment, and management of a statewide physical activity and nutrition (PAN) policy task force and county PAN coalitions; the management and support of statewide and county community tobacco prevention and control coalitions; the program management and coordination of statewide physical activity and healthy food access projects; and cancer coalitions and survivor networks. Such activities will assist the CDPHPD in supporting PSE change for physical activity, nutrition, and tobacco prevention and control, and the effective early detection and management of chronic disease to improve quality and years of life. For this RFP, “coalitions” are described as an alliance between private and public organizations and individuals whose overall goal is to influence long-term health. Coalitions are a mechanism for organizing and mobilizing communities to address complex issues systematically and comprehensively. The coalition should garner wide public support and increase the ability of the representative agencies to affect community policies and practices. The coordination among the members will minimize duplication and maximize resources and the community’s capacity to address the issues. The membership of the coalition should reflect the diversity of the community and the professional organizations that affect PSE changes.
