The selected Contractor will provide the following services: Employee Training for aerial lift users at each location that a lift is located, in compliance with OSHA and ANSI standards and any applicable state standards. Training Logs will be submitted via email to Jessica Allard – Environmental, Health & Safety Manager (Jessica.allard@jud.state.ma.us) • Yr 1 – Training for all lift using employees. • Yr 2 & 3 – Training for newly hired employees as needed. Annual OSHA and ANSI Inspections along with suggested inspections from Lift Manufacturer. Inspection Reports to be submitted at completion of lift inspection. Inspection Reports will be submitted via email to Jessica Allard – Environmental, Health & Safety Manager (Jessica.allard@jud.state.ma.us) Annual Preventative Maintenance service as recommended by Lift Manufacturer. Preventative Maintenance Reports to be submitted at completion of service. As needed Repair Service to lifts. Incidental repair costs incurred will be outside the scope of the service contract. Repairs are to be completed in a timely manner and by a trained and competent lift mechanic. All Repairs must be quoted prior to rendering repair service. Repair Reports to be submitted at completion of service. All parts used in Preventative Maintenance and Repair services shall be OEM or Equivalent parts.
