Services to be provided include, but are not limited to the following: Able to provide supportive services to 25-35 families, yearly. Able to enroll 3-5 new families, yearly Keep a waiting list of the families who cannot be accepted at the time of initial application or interest due to program capacity limits. Hold a scheduled [one on one or group] FSS informational and assessment for individuals/families showing interest in the program. Market and provide outreach on the FSS program through FSS flyers. Provide information during scheduled reexaminations Mail, email and make phone calls regarding FSS to eligible families once a year Complete an Individual Training Service Plan (ITSP) and signing of the contract of participation. Provide coaching services to help each participant family to: o Understand the benefits of participating in the FSS program and the how the program can help the family achieve its goals o Identify achievable, but challenging, interim and final goals for participation in the FSS program. Break down goals into achievable steps and accompany the family through the process o Identify existing family strengths and skills o Understand the family needs and connects them with supportive programs/services that may help the family make progress toward their goals o Access services available in the community through referral to appropriate service providers o Overcome obstacles in the way of achieving a family’s goals o Build partnerships with service providers o Help participants achieve other goals such as education, training, and preparing for homeownership
