1. The Neurologist shall perform the following services: • Evaluation of up to twelve (12) residents per day served by CHDC who are being considered for neurological medication or who have a neurological disorder or other neurological concerns and as ordered by the Primary Care Physician (PCP) as scheduled by CHDC Medical Transportation Staff. • Evaluation of up to twelve (12) residents per day served by CHDC for possible anti-epileptic drugs (AED) medication adjustments as scheduled by CHDC Medical Transportation Staff. • Routine medication checks, annually or more often as referred by the PCP, for residents who are receiving neurological medications. 2. For each of the above services, the contractor shall perform the following functions on an annual basis, at minimum, or as directed by the PCP: • Examination of the client. • Evaluating documentation such as lab tests results. • Staff observations. • Seizure frequency and duration. • PCP or other staff updates/concerns. • Consult with CHDC's medical, nursing, and/or team staff regarding the examination. The Neurologist will work with five (5) Physicians and Mid-Level Practitioners. Enter medication, lab recommendations, and progress notes as appropriate into the client's medical record. 3. Services shall be provided when scheduled by the HDC's scheduling personnel. The neurologist shall complete and sign the "Neurology Consultation" form or progress notes indicating results and recommendations.
