The owner is seeking proposals for maintenance of category 1 services. The requested service is for district-wide, self-provisioned fiber maintenance for repair and break-fix only. Routine, and/or annual maintenance is not being requested. The Respondent shall include in their bid price, at a minimum, the following: 1. The Respondent shall provide pricing for emergency maintenance (break-fix) for six (6) events per year. An event includes, but not limited to, cable repair including labor, splicing, splice cases, splicing consumables, and fiber optic cable. 2. The Respondent shall receive repair requests directly from the Owner. 3. The Respondent shall coordinate and furnish all labor, supervision, tools, materials, equipment (including vehicles, lifts, and trucks), necessary for the completion of all maintenance break-fix repairs in compliance with all local, state, and federal codes, ordinances, and accepted standards. 4. The Respondent shall provide sufficiently qualified staff, equipment and supplies to perform the duties outlined in this Request for Proposal. 5. The Respondent is responsible for obtaining all licenses, permits, inspections and other authorizations required for the contractor’s performance of the services. 6. The Respondent shall abide by all applicable OSHA regulations governing the safety of all employees, contractor vehicles, and all others at the work site. 7. The Respondent shall complete all work to the highest level of professional standards. 8. The Respondent must complete emergency maintenance in a timely manner and be compliant with industry standards and expectations. 9. The Respondent shall provide any requested maintenance and/or ongoing service during regular business hours whenever possible. 10. The Respondent shall provide emergency maintenance coverage 24 hours a day, 7 days a week, with a 4-hour response time.
