Provide services that will help decrease the risk for type 2 diabetes, HTN, and HBC among priority populations; and that will enhance quality of care and early detection of diabetes, HTN, HBC, and their complications. The Contractor will implement evidence-based approaches aligned with the Chronic Disease Control Branch’s Type 2 Diabetes Program and HeartBeatCA Program. The Contractor will provide training and technical assistance to T2DP and HeartBeatCA partners and to pharmacy networks in California. The Contractor’s trainings will include webinars and virtual meetings designed to educate and improve T2DP and HeartBeatCA partners’ skill and knowledge on various processes for integrating pharmacists into the health care team, such as how to implement agreements with pharmacists, education on best practices, and other procedures related to efficiency and process improvement for successful collaboration. Services will support federal investments for evaluating, assessing, and implementing evidence-based strategies to manage diabetes, HTN, and HBC, and improve health outcomes.
