3.1.1 PA Badge or equal 3.1.1.1 PA Badge or equal must be an Area Monitoring device. 3.1.1.2 PA Badge or Equal must provide radiation monitoring by way of Luxel. 3.1.1.3 PA Badge or Equal must come with Velcro Holder. Velcro must have adhesive backing for surface mounting. 3.1.1.4 PA Badge or Equal must meet all U.S. Accreditation Standards. 3.1.1.5 PA Badge or Equal must be heat, moisture and pressure resistant. 3.1.1.6 Vendor shall provide the Agency with approximately 165 PA Badges or equal on a quarterly basis. These will be shipped to the different locations (Exhibit B), known as “Sort and Pack”. If Additional Dosimeters are needed, Agency will notify the Vendor. 3.1.1.7 Agency shall pay for any unreturned dosimeter for each quarter. 3.1.1.8 Vendor must provide a NRC Form 5 Report or equivalent annually. See Attached Exhibit C NRC Form for example. 3.1.1.9 Vendor shall provide Agency with Online Access and Paperless Radiation Dosimetry Report. This shall be of no extra cost to the Agency.
