1. Clifton T. Perkins Hospital Center will provide a work area or exam room, refractor, ophthalmoscope, vision projector, examination chair with slit lamp and phoropter along with nursing staff for support services. The Contractor shall provide all other equipment and supplies necessary to provide the services. 2. The Contractor shall be responsible for providing optometry services in accordance with all applicable licensure, certification, and accreditation standards of State of Maryland, Department of Health, Office of Health Care Quality 3. Services to be performed under this contract include: a. Eye clinics twice a month during normal State business hours for eye exams, necessary treatments and adjustments. The clinics through shall not exceed four hours each. b. Comprehensive eye examinations from a Maryland State licensed optometrist, which includes possible Dilated Fundus Examination. i. The Contractor shall provide a written treatment plan on the Clifton T. Perkins Hospital Center consultation referral forms. ii. Each exam shall consist of the visual acuity tests and inner ocular pressure. c. Administrative and clerical services required for managing and coordinating services, rendering invoices, and performing other functions necessary for the management of the Contract. 4. Services to be performed under this contract include: a. Eye clinics twice a month during normal State business hours for eye exams, necessary treatments and adjustments, and repairs and fittings of eyeglass frames. The clinics shall not exceed four hours each. b. Comprehensive eye examinations from a Maryland State licensed optometrist, which includes measurements to ensure proper fitting of eyeglasses and possible Dilated Fundus Examination. i. The Contractor shall provide a written treatment plan on the Clifton T. Perkins Hospital Center consultation referral form. ii. Each exam shall consist of the visual acuity tests and inner ocular pressure. c. Providing standard, not designer style, eyeglass frames d. Providing fitting, repair, and routine adjustment of eyeglass frames on an as needed basis during scheduled monthly eye clinics. i. Once an eye clinic has been scheduled, eyeglasses that can be repaired on site by the Contractor shall be repaired at the time of the clinic. ii. If the eyeglasses cannot be repaired on site, the Contractor shall take the glasses back to their place of business to repair. e. Providing single, lined bifocal, and lined trifocal lenses based on the prescriptions written from an eye examination.
