The Contractor must provide professional and timely emergency residential placement services to the Department of Health & Welfare’s Division of Child, Youth, and Family Services (CYFS [Department’s]), Developmental Disability (DD) Crisis Prevention and Court Services (CPCS) Team in compliance with all relevant federal, state, county, city and Department laws, rules, and policies relevant to the services. All homes are subject to Department approval. The Contractor must: Maintain certification and be in compliance with all applicable IDAPA 16.04.17 Rules Governing Standards for Residential Habilitation Agencies. Comply with applicable Homeowner Association or equivalent covenants or rules applicable to the property. Ensure all service provisions are delivered by persons who meet provider qualifications outlined in IDAPA 16.04.17 and provide evidence of licensure, certification, and any other applicable provider qualifications to the Department on request. Maintain compliance with the Department's philosophy of least restrictive intervention. Deliver services in accordance with a written Crisis Plan or Safety Plan developed through a coordinated effort by the Department and the Contractor. Report to the Department immediately any critical incidents or any incident that has reasonable potential to negatively affect participant safety. The Department will provide contact information to the Contractor. Report to the Department and Child/Adult Protection Authority any allegations or disclosure of abuse or neglect within twenty-four (24) hours. The Department will provide contact information to the Contractor.
