EWEB RESPONSIBILITIES • EWEB shall notify Contractor, by phone, email and/or with an asphalt replacement form, when the service is required. • EWEB and Contractor shall work together to create a process to identify outstanding and completed work. This process shall be followed by both parties. • If flaggers will be necessary, EWEB may or may not provide EWEB Flaggers. EWEB has sole discretion of providing Flagging services as part of this contract. • EWEB will provide all clean up, tee, and final cuts prior to asphalt patching request. • If incidental asphalt or concrete cutting will be necessary, EWEB may or may not provide the asphalt or concrete cutting. EWEB has sole discretion of providing asphalt or concrete cutting. • EWEB is responsible for obtaining all permits and the contractor is responsible for adhering to all conditions and provisions in the permits. • If a Traffic Control Plan (TCP) is required, EWEB will provide the Contractor a TCP. CONTRACTOR RESPONSIBILITIES • Contractor shall provide Asphalt Concrete Patching services within 30 calendar days (unless agreed upon by both parties) of receiving asphalt patching request. It is the responsibility of the Contractor to inform EWEB if paving cannot be performed within the required time due to weather conditions and/or asphalt availability. Repeated failure to meet service timeline requirements may result in contract termination. • If flaggers are necessary and EWEB cannot provide this service, the Contractor will schedule and supply the flaggers per bid item of this contract. • ALL signing and traffic control devices will be the responsibility of the Contractor. Should EWEB leave its own devices in place for the Contractor’s use, this does NOT establish that EWEB will provide devices going forward. If Contractor uses EWEB devices, such as traffic cones and/or barricades, Contractor shall notify EWEB, within 24 hours that the devices can be picked up. Contractor shall be responsible to leave all traffic control devices in manner that insures the safety of the public. • Contractor is responsible for adhering to all conditions and provisions in all permits obtained for the project. (EWEB is responsible for obtaining all permits.) • Contractor will be required to submit a weekly, one week look ahead work schedule each Thursday morning (before 12:00 noon) when there is outstanding paving to be done throughout the duration of this contract. • Contractor shall be responsible for any spalling, chipping and/or cracking of any existing asphalt caused while performing their work. • If street plates have or need to be set and EWEB cannot provide this service, the Contractor shall provide the correct equipment to do so. • Contactor shall submit an invoice for work performed, per request. The invoice shall include material used per actual measurements of the asphalt concrete patched. • On occasion EWEB may require same day paving with a minimum of a 4 hr notice.
