The Contractor(s) shall prepare the Asphalt Mix for the designated delivery location, per the request of the SHA maintenance facility. The SHA maintenance facility shall inspect the delivery of Asphalt Mix upon each executed delivery. Upon the successful delivery and satisfactory inspection of the Asphalt Mix, the Contractor shall submit an Invoice to the State Highway Administration. Contractor shall provide SHA with the following information for billing purposes: A. Payment Information (reference to Standard Terms) 1. Name of Company, 2. Address to include the 9-digit zip code, 3. FEIN Number 4. The Company’s Contact person (including phone number, for placing orders) 5. Blanket Purchase Order referenced on the Invoice Failure to do so may result in the delay of payment.
