The City of Des Moines, hereinafter called the "Jurisdiction", has need of a qualified contractor to complete the work comprising the below referenced improvement. The undersigned Bidder hereby proposes to complete the work comprising the below referenced improvements or project as specified in the contract documents, which are officially on file with the Jurisdiction, in the Des Moines City Engineer's Office, at the prices hereinafter provided in Part C of this Proposal, for the following described improvements: 2025 HMA Residential Paving Program Contract 1, 062025002 The improvement includes furnishing labor, equipment, and materials for the removal of existing Hot Mix Asphalt (HMA) pavement and surfacing materials by milling/scarifying to an average depth of three (3) inches and four (4) inches, prepare subgrade/subbase, reconstruct streets with three (3) inches of HMA pavement in one (1) lift or four (4) inches of HMA pavement with two (2) lifts, replace sections of Portland Cement Concrete (PCC) curb and gutter, and replace existing sidewalk ramps with ADA-compliant ramps, all in accordance with the contract documents at the following locations and other locations as directed by the City Engineer in Des Moines, Iowa: Courtland Drive from Davis Avenue to Olinda Avenue E. 32nd Street from Indianapolis Avenue to Kinsey Avenue Thompson Avenue from E 27th Street to E 28th Street E. 28th Street from Skeies Drive to John Patterson Road Skeies Drive from E. 27th Court to E. 28th Street Alpha Avenue from E. 14th Street to Glenbrook Drive E. 15th Street from Alpha Avenue to Guthrie Avenue 24th Street from Forest Avenue to Clark Street 10th Street from Shawnee Avenue to NW Aurora Avenue Arapahoe Drive / John Lynde Road from 37th Street to 35th Street Havens Avenue from SW 14th Street to SW 12th Street
