The Contractor shall deliver mobile medical Supplies, delivery and training necessary to allow the Connecticut Early Detection and Prevention Program (CDEPP) an integrated program consisting of the CT WISEWOMAN Program and CT Breast & Cervical Cancer Early Detection Program provider sites throughout the State of Connecticut and their associated intake centers to carry out Minimum Data Elements required by the Centers for Disease and Control and Prevention including height, weight, blood pressure, body mass index blood work to include: total cholesterol, high-density lipoprotein, low-density lipoprotein, triglyceride levels, fasting glucose & Hemoglobin A1C. Contractor employees Performing work under the Contract shall be a navigator of the CDEPP. Navigator of CDEPP can be one of the following: a Clinical Navigator (Registered Nurse or Social Worker), a Health Systems Navigator (or site coordinator for the program provider sites), and a Community Health Navigator (functioning as a Community Health Worker, able to bring program Services to the community settings where program participants work and live). The CDEPP navigators shall work together as a team to best navigate the program participants to the care they need.
