Based on the learning objectives for the conference a keynote speaker should have a focus on at least one of the conference objectives, listed below, with higher priority on a speaker that can address more than one. Learn how addressing social determinants including health literacy improves health outcomes. Explore how using data effectively and responsibly can drive patient centered care and health equity. Examine strategies to integrate behavioral health into primary care for individuals of all ages. Identify innovative approaches that leverage the Health Care Homes (HCH) model to improve patient and team member experiences. Examine evidence-informed methods for empowering patients in chronic disease prevention and management. Draft of presentations to HCH for review and comment by April 24th. In addition to the conference objectives the speaker should demonstrate: Understanding the Minnesota’s Patient-Centered Medical Home Model - Health Care Homes. Support primary care and integration of whole person care. Have experience/knowledge of health equity and specifically how primary care can advance health equity. Priorities for the keynote Relevant to Minnesota. Include “how-to” of ideas presented (strategies that can be implemented). Materials/resources to take-away. Have some familiarity with the national healthcare landscape to be able to make connections between our work in MN and the alignment/opportunities at a federal level. Be able to set the tone for the day – welcoming and encouraging/energizing, get participants excited for the day ahead and for the work they are doing. Connection or experience in doing the work. Responders are encouraged to propose additional tasks or activities if they will improve the results of the project. These items should be separated from the required items on the Cost Proposal.
