A. To provide the necessary services to properly screen and recoat wood athletic floors at campuses identified in Schedule A above. B. Each Vendor shall provide evidence of being in the gymnasium screening and finishing business for no less than three (3) years and shall have all the necessary licensure and certifications. C. Each Vendor shall submit a list of three (3) references for which the Vendor has provided the same services. References will include current contact names, addresses, telephone numbers, and date when services were provided. D. The Vendor may perform on-site visits to the listed facilities in order to become familiar with the existing conditions. Failure to perform on-site visits does not relieve the Vendor from any performance responsibility. The Vendor shall coordinate with the District Representative prior to any site visits. E. Each Vendor shall be responsible for fully completing the attached Proposal form.
