- Provide routine services to the pool (s) at the request of the MPRB Project Manager (PM) - Provide as requested on-call services to support repairs for the pools, as requested by the MPRB PM or their designee; labors must include name, classification, hourly rate, and hours worked. - Provide pool chemicals at a preferred discount off list price; supplies/chemical charges must include a discount of the list price. - Vendor must invoice monthly for services rendered. Pre-payment is not allowed. Invoices must be detailed line-itemed invoice including: i) Facility name and address ii) Nature of work iii) Labor (name, classification, hours) iv) Parts, supplies or chemicals used for project v) Contract PO number to be included on the invoice
