The purpose of this Request for Proposal (RFP) is to solicit competitive sealed proposals from qualified suppliers to provide Canyons School District (CSD) students Limited Services (assessment and UP TO SIX [6] sessions),approved and determined by Canyons School District and mental health treatment such as: individual/group/family counseling, crisis intervention, and assessment. Services are based on the availability of grant or donated funds through the CSD Foundation. Services must be initiated within 60 days of the date of referral noted below. Parent/Guardian(s) will be responsible for any cancellation fees to the provider in the event they do not show up for a scheduled appointment. It is anticipated that this RFP may result in a contract award to a single or multiple suppliers for services as needed, as defined further in the Scope of Service.
