The NHJB seeks to contract with an entity for the purpose of providing case management services to parents in the Family Treatment Court in Sullivan County beginning on or after June 1, 2025. The selected vendor must provide the following: Personnel • A dedicated case manager to provide case management for the FTCSC parent participants. Case management will include daily communication between the case manager and FTC team members, including court personnel, and the parents. It will also include managing information and working with entities that provide services to the child(ren) involved in the abuse/neglect case. • Ability to provide case management services county-wide to include coordination of drug screenings; support of parents and team to access treatment; track and document individual and family treatment progress; support participating parents in completing court assigned tasks; and maintain regular data collection on case information for entire family. • Personnel free of conflicts of interest with entities involved in FTC. Standing entity partners include: Division for Children, Youth, and Families (DCYF); the Court; CASA; individual parent attorneys; and TLC Recovery Resources. Any conflicts should be addressed in proposal. • The case manager and supervising entity may provide some treatment services, depending on skill and training of the case manager and the needs of the FTCSC parents, including assessed ASAM Levels of Care. The case manager will be responsible for ensuring ethical, appropriate referrals.
