1. Provide curb-to-curb transportation for older persons who require help getting from one location to another, using a vehicle, based on the Order of Priority. 2. Coordinate special travel arrangements as feasible and as authorized. Have other transit options available for disabled elderly. 3. Provide curb-to-curb assistance, as needed with the support of a helpful driver, to: a) Ensure the safety of passengers from inside the place of departure to inside the place of destination. b) Assist clients to negotiate from their doorway into the vehicle and to disembark from the vehicle, at pre-agreed locations. c) Load and unload clients and their assistive devices, according to schedule. d) Assist clients with mobility limitations to assure they do not fall and can manage stairs, ramps, curbs, or other possible impediments. 4. Provide transit options for persons with disabilities. 5. Accept authorization for delivery of service issued by the AAA. The provider must check for any authorizations with the Management Information System (MIS), required by the EOA at least once every business day, unless directed otherwise by the AAA. 6. Record and submit the service units delivered to each client’s record using the MIS required by EOA, unless directed otherwise by the AAA. 7. Provide observation and feedback to supervisors, primary caregiver, and EAD, on changes in client’s health and social situations. 8. Record any other relevant information into the client’s record. 9. Collaborate and coordinate with other service providers that serve the same client, as appropriate.
