Non-Physician Practitioners (NPPs) shall provide services to the veterans at the Thomson-Hood Veterans Center (THVC), which includes the evaluation and management of patient care for both scheduled and acute encounters. THVC regularly utilizes up to three full-time equivalent NPPs within its organizational model. These requirements are met through a combination of facility-based NPPs and contract service NPPs. NPP Services must adhere to all relevant state and federal guidelines and regulations that pertain to skilled nursing facilities. The NPP will also comply with regulations set forth by the Centers for Medicare and Medicaid Services (CMS), and provide care within their scope of practice, which includes but is not limited to the following duties: # Conduct nursing facility rounds as needed, in accordance with applicable regulations. # Utilize PointClickCare software for patient management and documentation # Participate in the existing call service rotation (approximately 1-2 weeks of on-call service per month) # Coordinate emergency care for THVC residents who may be admitted to a hospital. # Facilitate the transport of THVC residents to the nearest acute care facility when necessary. # Oversee and manage the care of THVC residents in a skilled nursing facility setting, including supervising their medical care.
