This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
The Solicitation number of this Request for Quote (RFQ) is 285-21-RFP-0031. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-04 January 25, 2021. This is a combined synopsis/solicitation is 100% set aside for small business enterprises. The associated NIACS code is 562720 Janitorial Services and the size standard is $19.5 million.
Products and services to be provided by the contactor:
The contractor will work on Monday, Wednesday and Friday afternoons from 4:30pm until 8:30pm. For a total of 12 hours a week. In some instances there may be more hours needed. The location services will be provided is 2893 Sturgis Rd.

The contractor shall ensure that the waiting room, visitor bathrooms, patient registration area, lab bathroom, all 7 patient rooms, staff breakroom. Staff bathroom, provider’s office, nursing office, staff kitchen, all hallways, nutrition room, pharmacy room, pharmacy consultation room, all dental rooms and 2 dental offices, 4 administrative offices are swept and mopped.
The contractor shall ensure that the waiting room, visitor bathrooms, patient registration area, lab bathroom, all 7 patient rooms, staff breakroom. Staff bathroom, provider’s office, nursing office, staff kitchen, all hallways, nutrition room, pharmacy room, pharmacy consultation room, all dental rooms and 2 dental offices, 4 administrative offices have trashed emptied and clean trash bags in the trash receptacles and in the recycle bins in each area.
The contractor shall ensure that the waiting room, visitor bathrooms, patient registration area, lab bathroom, all 7 patient rooms, staff breakroom. Staff bathroom, provider’s office, nursing office, staff kitchen, all hallways, nutrition room, pharmacy room, pharmacy consultation room, all dental rooms and 2 dental offices, 4 administrative offices have all window seals wiped out and dusted, along with any window blinds.
The contractor shall ensure that the waiting room, visitor bathrooms, patient registration area, lab bathroom, all 7 patient rooms, staff breakroom. Staff bathroom, provider’s office, nursing office, staff kitchen, all hallways, nutrition room, pharmacy room, pharmacy consultation room, all dental rooms and 2 dental offices, 4 administrative offices have all chairs, tables, counters, toilets, sinks and furniture wiped down, dusted and sanitized.
In the event that staff shortage service unit shall contact the contractor by 12:00pm and they should ensure that someone will be on site by the next morning at 8 a.m. to do the duties listed above
The contractor shall have cleaning supplies and trash bags supplied by the Catawba Service Unit.
The Period of Performance shall be One Base Year, plus Four Option Years. Base Year period of performance starts 10/01/2021

Contractor understands that the government does not pay any IRS requirements (taxes), insurance or any other benefits for the contractor under this contract. This is in accordance with (FAR 52.229-3 FEDERAL, STATE, AND LOCAL TAXES (FEB 2013) and FAR 52.229-4 FEDERAL, STATE, AND LOCAL TAXES (NON-COMPETITIVE CONTRACT [FEB 2013]).
