Specifications include, but are not limited to: Mental Health Assessments: Vendor will complete a Mental Health assessment on one hundred (100) percent of individuals referred and provide a written individual assessment identifying needs and recommended services to be completed and provided to TCVTC within ten (10) working days of appointment. A. Mental Health Evaluation − Written evaluations conducted by a licensed professional to assess a veteran’s overall emotional, physical health, cognitive functioning and identify needs. The budget for this section is $40,000.00; Intensive Outpatient Program: Vendor will provide JIV’s treatment based on an individualized treatment plan that will be documented and provided to TCVTC within ten (10) days of first appointment. Monthly documentation on JIV’s level of participation and compliance with treatment goals and objectives progress or lack of progress will be provided to TCVTC by the 5th of each month for all services provided to JIV’s by Vendor/Vendors. A. Intensive Outpatient Program (IOP) − May be recommended for those who do not need medically supervised detox. 48 Group Sessions and 24 Individual Sessions. The budget for this section is $14,400.00. B. Supportive Outpatient Program (SOP) − A treatment program that uses clinical treatment methods to help make positive changes in the participants’ life. 24 Group Sessions and 12 Individual Sessions. Each JIV exiting treatment will have a discharge plan completed and forwarded to the TCVTC within three (3) days of the JIV’s discharge. The budget for this section is $97,200.00.
