Specifications include, but are not limited to: A. Mental Health Evaluations: Vendor will complete a Mental Health Evaluation on 120 individuals referred from the TCVTC and provide a written individual evaluation identifying needs and recommended services to be completed and provided to TCVTC within ten (10) working days of appointment. 1. Mental Health Evaluation − Written evaluations conducted by a licensed professional to assess a veteran’s overall emotional, physical health, and cognitive functioning and identify needs. The TCVTC will pay no more than $400.00 per evaluation. Mental Health Evaluations will include treatment recommendations, individualized treatment plans, and the following screening assessments: • Clinical Interview and Psychosocial History • PCL-M (Post Traumatic Stress Disorder Symptom Checklist) • HAM-A (Hamilton Anxiety Scale) • BDI-II (Beck Depression Inventory • BHS (Beck Hopelessness Scale) • SASSI-4 (Substance Abuse Subtle Screening Inventory) • Personality Assessment Inventory (PAI) B. Outpatient Program Treatment: Vendor will provide JIV’s treatment based on an individualized treatment plan that will be documented and provided to TCVTC within ten (10) days of first appointment.
