The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources, small and other than small businesses, to perform a potential requirement. NOTE: Respondent claiming SDVOSB and VOSB status must be certified in US Small Business Administration s VetCert System, https://veterans.certify.sba.gov Project Requirements: NCO 17 on behalf of the El Paso Texas Veterans Healthcare System, Healthcare for Homeless Veterans (HCHV) Program Office located at 5001 North Piedreas Street, El Paso, TX 79930-4221 is conducting market research to find available contractors within the greater El Paso, TX area to provide residential placement and supportive services through the Health Care for Homeless Veterans (HCHV) program for up to 20 homeless Veterans daily. Contractor shall serve homeless Veterans in need of immediate shelter in a safe environment. Many of these veterans have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. Contractor shall provide rapid placement in a supportive, residential setting to VA referred Veterans. This housing must meet the criteria established both by HCHV clinical staff and VA fire and safety officials. Place of Performance: Services shall be provided off site at the Contractor s location. Anticipated period of performance: Base plus four (4) one-year options beginning July 2025. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s). Synopsis SERVICES REQUIRED. The purpose of this RFP is to engage a community provider (contractor) who will provide transitional housing for an estimated up to 20 Veterans along with services consistent to meet HCHV program objectives. The goal of this contract is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, emergency housing services to provide clinical case management with supportive services, life skills training, sobriety maintenance and mental health recovery education, vocational and housing counseling services to aid the veterans in reaching goals of attaining permanent housing successfully while in the program and maintaining stability. PLACE OF PERFORMANCE. Services will be provided in Contractor-provided facility. Facility must be located in El Paso, El Paso, Texas and must be appropriate for providing a safe, residential setting while addressing Veterans complex needs through on-site case management and therapeutic support services. Contractor shall have existing programs and facilities in El Paso, Texas that is fully operational and can start providing services within 10 days of contract award. The Contractor shall be expected to provide safe emergency community-based residential housing and quality services for homeless Veterans referred by the Health Care for Homeless Veterans (HCHV) program staff. Goal is to provide services to enhance the stability of homeless Veterans served by HCHV. Housing will be available 24/7, 7 days a week. Placement can be up to 90 days with an extension based on approval by the program contact. The goal is that a minimum of 65 percent of Veterans will successfully discharge to independent living in the community. License and Certification. The Contractor must have site control of the housing facility through ownership or a valid lease. Contractor facility will be certified by the State of Texas, Department of Health and Human Services, Division of Health, Bureau of Licensure and Certification as a facility for transitional housing. The Contractor shall meet all City, County and State requirements concerning licensing and health codes. The Contractor s facility shall meet applicable requirements of the Joint Commission on Accreditation of Hospitals, reference at Hospital Accreditation | The Joint Commission. The facility shall be wheelchair accessible and meet Americans with Disabilities Act guidelines for buildings and facilities found at http://www.ada.gov/reg3a.html#Anchor-Appendix-52467 Contractor shall be responsible for providing sufficient residential capacity to place up to 20 Veterans in safe community-based residences. Suitable emergency shelter placement generally requires: Sufficient personnel to assure security Staffing available on-site 24 hours a day, 7 days a week 3 healthy meals per day Laundry facilities and detergent available for Veteran to do their own laundry The ability to place a Veteran on the same day of a referral from HCHV staff Hygiene products to include wash cloths, soap, feminine products, deodorant, toothpaste, and toothbrush shall be made accessible to veterans. Transportation to contracted facility upon admission when needed by veteran from key locations, such as the El Paso VA Medical Center. Transportation to VA Medical appointments or vouchers for use of public transit. Suitable bedroom space and furnishings allowing for adequate storage and security of belongings, with minimum 140 square feet of living space for two veterans. Services which the contractor must be able to furnish include: Contractor to provide area in the facility for VA Staff to conduct Individual or Group treatment session with Veteran s based on Veteran s treatment plan. Collaboration with appropriate VA staff to insure linkages to and provision of supportive psychosocial services. Support for an alcohol/drug abuse-free lifestyle, with witnessed urine drug and alcohol screening scheduled regularly and random of veterans. Contractor shall transport a Veteran in a mental health crisis to appropriate facility if needed. Contractor shall provide appropriate recreational activities to Veterans placed in facility. The Contractor shall assign to this contract personnel that by education and training (and, when required, certification or licensure) are qualified to provide the services as denoted in the Performance Work Statement.
