Specifications include, but are not limited to: A. Catalogs: It is mandatory that each vendor submits, with their proposal, two (2) copies of the most current published catalog, available, and maintain pricing updates as they change for the duration of the contract. Proposals received without catalogs will be disqualified. Vendors should include information regarding on-line availability of catalogs if appropriate. B. Prices: Parker County is asking for a percentage above/below the AWP (average wholesale price). The percentage above/below the AWP is the price Parker County would pay for each prescription and non-prescription items, whether national brand or generic. Generic prescriptions should be used, unless no generic exists. A formal proposal form has been provided with the RFP. All vendors are expected to write-in their percentages on their formal proposal form. A space for showing your percentage is provided for prescription drugs (name brand and generic) as well as non-prescription items (name brand and generic). The percentage above/below the AWP must include any dispensing fees, delivery and freight charges. Parker County is exempt from State Sales Tax and Federal Excise Tax. Therefore, such taxes must not be included in proposed price. Invoices: All invoices must be addressed directly to the Parker County Jail, 612 Jamison Street, Weatherford, TX 76086. All invoices are to contain the following information: Prescription Drug Information: a. Inmate Name b. Name and description of the prescription dispensed c. Prescription Number d. Eleven digit NDC number e. Date description dispensed f. Quantity dispensed g. Average Wholesale Price h. Parker County’s Price Non-prescription items: a. Name and description of the item b. Average Wholesale Price c. Parker County’s Price C. Computer Capability: Each vendor must have the capability of tracking, through a computerized system, patient profiles, to determine, at any time, the eligibility and or approval, of a patient, for county benefits. The successful Vendor’s representative(s) will work with various county personnel to assure quick processing, delivery and payment.
