Responses to the information requested will assist the Government in determining the appropriate acquisition method. VHA RPO West NCO 17 Health Care Resources Branch is planning to solicit for a vendor who can provide on-site interventional radiology physician services. Description of Services: The Contractor shall provide Board Certified Abdominal/GU, Breast Imaging, Cardiovascular, General, Interventional, Muscoskeletal, Neuroradiology Radiology Physician Services on site in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the South Texas Veterans Health Care System (STVHCS). The VA Radiology Services follow the standards & guidelines set forth by American College of Radiology (ACR). A contractor providing onsite Abdominal/GU, Breast Imaging, Cardiovascular, General, Interventional, Muscoskeletal, Neuroradiology radiologist services shall provide services that meet or exceed the American College of Radiology Guidelines https://www.acr.org/-/media/ACR/Files/Practice-Parameters/IRClin-Prac-Mgmt.pdf.pdf (and are accredited by ACR for Mammography. Place of performance: Department of Veterans Affairs South Texas Veterans Health Care System Radiology Service 7400 Merton Minter Street San Antonio, TX 78229-4404 VHA RPO West NCO 17 Health Care Resources Branch is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to fulfill this acquisition. Type of Contract: If this sources sought notice leads to a future solicitation, the Government intends to award a fixed price indefinite delivery indefinite quantity (IDIQ) contract. The performance period shall be one (1) base year plus four one-year options. The proposed NAICS Code is 621111, Offices of Physicians (except Mental Health Specialists). Interested parties shall provide a brief overview of their firm's capabilities and experience with projects similar in scope to this project. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each capability statement submittal shall also include the following business information: a. Organization Name b. Organization Address c. Organization SAM Unique Entity ID Number d. Socio-economic status (IF SDVOSB or VOSB - provide SDVOSB or VOSB verification) g. Whether your company has experience providing interventional radiology physician services. Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 621111.
