Work): Department of Workforce Services (DWS), Office of Child Care (OCC) will contract with Brookes Publishing to assist Utah in universalizing developmental screening practices across Utah mixed early care and education service delivery model using the proprietary Ages and Stages Questionnaires, Third Edition (ASQ-3) and the Ages and Stages Questionnaires, Social and Emotional, Second Edition (ASQ:SE-2) to screen children birth through age five using their online platform and screening tools and resources.; Brookes will provide DWC-OCC annual subscriptions for ten (10) ASQ Online Enterprise accounts. DWC-OCC will be able to add programs to the ASQ Online Enterprise for all physical sites at which DWC-OCC offers early childhood care and education. UDWF will be able to access ASQ -3 in English and Spanish, and ASQ:SE-2 in English and Spanish, in the ASQ Online Enterprise account using keycodes located in the ASQ products they purchase or already own. Brookes will also allow access to downloadable PDF copies of the ESQ (Environmental Screening Questionnaire); Brookes will provide DWS-OCC annual subscriptions for ten (10) ASQ Online Family Access accounts for the ASQ Enterprise account holders to give the option of having parents or caregivers complete questionnaires via the internet.; Each ASQ Online Enterprise account will have an ASQ Online API (application programming interface) subscription with access to tokens to use to move child record and/or scoring data to other database management systems or applications. Subscription rate is outlined in the attached Budget. Subscription rate in the first year includes initial set up.
