The State of Vermont is interested in obtaining bids to meet the following business need(s): Inpatient and Outpatient Rehabilitation Services along with Restorative Care Nursing Services for the Vermont Veterans’ Home. 2.2. The successful bidder will provide rehabilitative services to the Home’s Veterans and members at least 6 days a week. 2.3. The bidder must meet and/or exceed generally accepted standards of practice for the provision of rehabilitative services. 2.4. Provide physical, occupational and speech therapy services in accordance with the attending physician’s orders and treatment plan. 2.5. Work with other clinical programs in determining care planning and treatment modalities. Therapists will assist the Home in complying with all MDS regulatory requirements. Therapists must recognize that they have an important role in ensuring an appropriate Medicaid/Medicare Case Mix for the Home. 2.6. Assess resident progress and response to treatment. Participate, as necessary, in care plan reviews with the interdisciplinary care plan team. 2.7. Provide home assessments for potential discharges and report outcomes to the interdisciplinary team. 2.8.
