One (1) yellow 28 pound Kit Envelope measuring approximately 9” x 12”. The specified text and graphics on the Kit Envelope shall be printed in black ink (See Attachment A). One (1) “Integrity Seal” sticker measuring approximately 3” x 1.5”. The following information shall be printed on the sticker: Integrity Seal TO BE OPENED BY AUTHORIZED PERSONNEL ONLY. See Attachment F for a generic example of the layout and type of information to be printed on the “Integrity Seal.” The color may be black text on a red background or red text on a white background. Envelope shall be sealed with integrity seal after contents are placed inside. 2. Each individual kit shall also be assigned a unique Kit Number with the same unique Kit Number in barcode language below the Kit Number. The unique Kit Number (and corresponding barcode language) shall be in the format “NFKXX-XXXXX,” where the first three numerical digits immediately following “NFK” represent the current year (“225” for 2025), and the last five numerical digits represent the assigned unique kit number. The lot number and Kit Number may be printed directly on the Kit Envelope. See Attachment A for the required placement of this information.
