Specifications include, but are not limited to: A. Provide phone, fax, and text options to receive calls and referrals related to the populations described in Section 1.0, above. The phone, fax and test line(s) shall remain unchanged over the contract period and be publicly advertised in collaboration with community partners. B. Provide care navigation. Care navigation ensures the effective and timely connection of families to health and social services. Care navigation is a service provided primarily by phone or text, with in-person accommodation available when requested. Care navigation line will be staffed, at minimum, Monday through Friday from 9:00 AM to 5:00 PM. Care navigation activities include: 1. Provide brief screening and assessment of the child and family’s needs to determine appropriate referral. 2. Assist families to understand and access community services for which they or their child may be eligible, including resources to address basic needs. 3. Assist families to complete referral paperwork, as needed. 4. Accept referrals from health and social service providers, with consent from the family. 5. Assist families to coordinate care for their child amongst multiple health and social providers. 6. Assist pregnant and postpartum individuals to access resources related to perinatal mental health. 7. Follow-up with families and referral sources as appropriate, to ensure necessary connections are made. Identify during follow-up if additional needs have arisen.
